Home and Community Care requires us to provide information about our activities and clients for their national data repository. This is to enable them to make decisions to provide better services where they are needed. All information passed onto Home and Community Care is completely anonymous. No one's full name, address, or any other identifying information is given out. You can ask to be excluded from the statistics that are given if you want to. Please remember the more information we give Home and Community Care, the better equipped they will be to make decisions about future services that can help you in the community.
All client records are kept securely in a locked filing cabinet or password-secured computer. Information stored is restricted to relevant personal information and notes on client care.
You, or, where relevant, your next of kin, advocate, or legal guardian can always request to see your personal file. While this information is not available to others without your consent, the information concerns you personally and you are entitled to view to it at any time by ringing the Home and Community Care Coordinator on (07)3399 7755 to make an appointment to access your information. The contents of your file are the property of Balmoral Uniting Community Centre and may not be removed from the Centre.
